ขอคำชี้แนะ เรื่องฟอร์ม RB1TO2 ออกบัตรใหม่

อยากจะเ้พิ่ม นามสกุล บิดา และมารดา  Fatherlname , Motherlname
ในการออกบัตรใหม่ ฟอร์มไม่โชว์ นามสกุลของบิดาและมารดาครับ
มีแต่ชื่อ....รบกวนด้วยครับ....
